Please, fill this Form in BOLD CAPITAL LETTERS and return it with your Registration Fee, photocopies of all your educational certificates, 2 recent passport-size photographs with your signature at the back, and 2 office flat files.

NOTE: 

1.  To transfer credits from your previous academic work, request all schools beyond Secondary school to send your official transcripts 

     directly to the Seminary for evaluation.

2.  Request your senior Pastor and 2 other matured believers who have known you for at least 2 years to send a letter of recommendation

     directly to the Seminary testifying to your new birth experience, Christian character and call to the ministry.
